Most people who ask about microneedling are really asking about the marks acne left behind, pores that photograph larger than they look in the mirror, skin that stopped bouncing back. It is also one of the most misunderstood treatments, because a cheap roller sold online borrowed the same name.

The pen drives needles straight in and straight out, at roughly 1,920 micro perforations per second. A roller drags its needles in at an angle, widening each entry point and increasing trauma for the same nominal depth. The literature includes a reported tram track scarring effect linked to rollers more than pens.

Dermatologists classify skin by the Fitzpatrick scale, how skin behaves in the sun: type IV is olive to light brown, type V is brown, type VI is deep brown to black. A large share of Miami, and of our patients, sits in that range, where the dominant complication is post inflammatory hyperpigmentation, the dark mark that outlasts a pimple by months.
Lasers and intense pulsed light deliver energy to a target that absorbs it, and melanin absorbs light very efficiently, so more heat lands where you did not want it.
After ablative CO2 laser resurfacing, post inflammatory hyperpigmentation has been reported in the large majority of patients at type IV and above.
Nonablative fractional resurfacing is gentler, but self limited hyperpigmentation is still common at higher treatment densities. Microneedling changes the equation because there is no chromophore and no thermal energy. Comparative studies against fractional CO2 laser for atrophic acne scars agree: the laser can be somewhat more effective per session, at the cost of longer healing and more post procedure hyperpigmentation, while microneedling shows a lower incidence, in some series essentially none, in types IV to VI. For a patient with brown skin and acne scars, that tradeoff usually resolves in favor of microneedling: slightly slower, much less likely to leave a new problem on top of the old one.
Safer is not risk free. Keloid history is an absolute stop, and tram track scarring has been reported in darker skin. In melanin rich skin we still prepare with daily broad spectrum sunscreen and, when appropriate, a few weeks of pigment calming topicals before deeper work.
What it treats well, and what it does not
Realistic targets
Atrophic acne scars, particularly rolling and boxcar depressions, overall skin texture, enlarged looking pores, fine lines. Stretch marks need deeper settings and more patience, and newer reddish striae respond better than old white ones.
Not the right tool
Microneedling adds no volume, so it will not replace filler for hollow areas or correct meaningful sagging. Deep ice pick scars usually need subcision or TCA CROSS from a dermatologist, active cystic acne needs medical treatment first, and melasma (paño) is a special case that belongs inside a full pigment plan rather than as a standalone fix.
